progressive hearing loss in right ear

Findings:


  • Axial HRCT temporal bone images show focal lucency in the region of right fissula ante fenestram. 

Diagnosis:

Otospongiosis


Discussion:

  • Causes progressive hearing loss, commonly presenting in 4th or 5th decade.
  • Most common type involves fissula ante fenestram (anterior to oval window). Other type is retrofenestral which involved the boen surrounding the cochlea.
  • Typically occurs as lucent bony changes throughout most of its clinical course, hence the term otospongiosis. Also called otosclerosis as late stage involves sclerosis of the involved bone.
