h/o long standing arrhythmia and amiodarone use

Findings:


  • Axial HRCT temporal images show subpleural interlobular septal thickening in upper and lower lobes of both lungs.
  • Pacemaker leads in ventricles
  • Homogeneous liver hyperdensity

Diagnosis:

Amiodarone deposition in liver

Amiodarone induced pulmonary fibrosis (presumed)

Discussion:

  • Amiodarone is an antiarrhythmic medication which can get deposited in the liver and lungs.
  • In the lungs it can cause interstitial lung disease in the form of chronic eosinophilic pneumonia, diffuse alveolar damage or organizing pneumonia. It can also cause pulmonary fibrosis.
  • Amiodarone deposition in liver appears hyperdense because of its high iodine content. However it is usually asymptomatic and hepatic dysfunction as a result of it is uncommon.
