h/o abdominal pain and swelling

hydatid cyst iliacus CT

Findings:


  • Axial contrast enhanced CT shows a multilocular cyst with multiple internal non-enhancing daughter cysts in the right iliacus muscle.


Diagnosis:

Hydatid cyst of iliacus muscle


Discussion:

  • Musculoskeletal hydatid cysts are a rare form of hydatid infection.
  • Common locations for MSK hydatid include vertebrae and paravertebral muscles, pelvis and lower limbs. 
  • If unilocular other cystic appearing lesions may be added to the differential such as intramuscular myxoma, chronic hematoma or nerve sheath tumor.
