MSK and sports case 4

h/o anxiety and pain in temporal regions

Findings:


  • Coronal and Axial T2 images show symmetric significant hypertrophy of bilateral masticator muscles.
  • No lesion or edema within the muscles.


Diagnosis:

Bilateral masticator muscle hypertrophy (Minotaur syndrome)


Discussion:

  • Masticator muscle hypertrophy is a rare condition which predominantly affects temporalis and masseter muscles, and can be unilateral or bilateral. 
  • It can be seen in anxious individuals who do teeth grinding (bruxism) or those with a history of long standing gum chewing. 
  • Role of imaging is to exclude myositis or tumors.
  • It is named after the Minotaur in Greek mythology who is portrayed as having  the head of a bull and body of a man.
